I will review case submissions for discussion during the meeting. I prefer either cases definitely better or cured, or one well managed (though perhaps not a curable situation).

Depending on how many I get it may be possible also to work through a case you are having difficulty with.

Submissions should reach me by January 15, 2010. After that time, check with me as to schedule availability.

If by credit card, you will be sent an invoice from Paypal by email so you can complete the payment. You need not register as a Paypal member to make this payment, just follow the email instructions.

If you do not have an email address, then you must pay by check.
LODGING & MEALS PLAN *
Single room & meals ($188/ night). Double room & meals ($156/night). Triple or Quadruple room & meals ($138/night). Day use & lunch only ($25/day). Children, 7-17 ($88/night). Children, 3-6 ($38/night). Children under 7, free.

The day totals and 4-day totals are rounded off. The total prices for all 4 days is:
Single = $749.
Double = $624.
Triple/Quad = $550.
Day use only = $100.
Children, 7-17 = $350.
Children, 3-6 = $150.

We will make final room assignments on Feb. 15 and bill you for the amounts.
